Forms & Applications

Exemption applications must be submitted no later than April 1st. They may be submitted beginning Sept 1st. The earlier they are submitted, the earlier they are reflected on your tax bill.

The Form of List (FOL) must be filed annually. M.G.L. Ch 59 Sec 2 states that all real and personal property are subject to taxation. All persons or entities owning locally valued taxable personal property on January 1st are legally required to file a Form of List with the assessors. This applies to second home owners and most businesses. The Form of List is due on or before March 1. Taxpayers should provide original cost and date of acquisition.

Chapterland  61 A & 61 B applications must be filed yearly and are due in the assessor's office no later than October 1st at 1:00 pm. Applications not timely received will result in the propertey's removal from the program.

  • Motor Vehicle Excise Abatement Application
    Bill of sale and plate return receipt from the RMV or new registration form if plate transferred to another vehicle must accompany this application
  • Real Estate or Personal Property Abatement Form
    According to MGL CH 59, these forms CAN NOT be submitted until the actual tax bills are mailed; usually no later than Dec 31st. Forms are due 30 days from the date the actual bills are mailed; usually Feb 1st.
